Submit copay claims for in-office administered therapy.

To submit a medical copay claim for SPINRAZA^® (nusinersen), TYSABRI^® (natalizumab), or other Biogen buy and bill products, you will need:

• Explanation of Benefits (EOB) or Claims Remittance Advice (EOP)
• Documentation provided via portal must include:
  • Patient cost share for the Biogen drug covered in the program
  • Patient cost share for the administration fee related to injection or infusion of the Biogen drug covered in the program
  • Named patient who is covered/eligible for the Biogen Copay Program
  • Biogen product name or associated J-codes
